Through these type of assistance seniors continue to live in the comfort of their home with support and maintaining their dignity and independence. These services are offered to seniors who live within our catchment area. This area is defined as: from Steeles to highway 401 and from Victoria Park Avenue to Bathurst street.
NYSC Community Support Services include:


North York Community Care Access Centre Personal Support

Clients for this service are referred to our program through the North York Community Care Access Centre (NYCCAC). Clients receive assistance with personal care and light household duties if needed. NYCCAC determines clients’ eligibility for assistance and assumes the cost of this service.

Respite Care for Caregivers

A support service designed to relieve caregivers stress. A qualified Personal Support Worker will attend to the client in the privacy of his or her own home. Time can be spent interacting with the client while attending household needs.

An NYSC Case Coordinator will complete an assessment in the client’s home to identify the specific needs of the prospective client.

Location: In the client’s home.

Schedule: The NYSC Case Coordinator will arrange a time for the in home assessment and for the provision of Respite Care directly with the client and/or the caregiver.


Caregiver Support Groups and Education Sessions

Provides emotional and educational support to caregivers to help them cope with the day-to-day stress related to the provision of care to a senior.

Group peer-support sessions, educational workshops and individual support are available on a regular basis and as required by the caregiver. NYSC’s Caregiver Support Groups are open to all community caregivers regardless of whether the recipient of care is a participant of NYSC services.

Location: 80 Sheppard Avenue West.

Schedule: Monthly, every 2nd or 4th Thursday morning. Monthly, every 3rd Tuesday, in Cantonese.


Transportation Services

Transportation is available to facilitate NYSC’s members’ and clients’ participation in NYSC’s programs and services.

Transportation is available to: -Users are members of the Senior Adult Centre or are accessing Adult Day Services

-Users are registered with the Transportation Department

-Users reside within NYSC service catchment area

-Users are unable to use public transit because of physical or cognitive limitations

-Users are not eligible for Wheeltrans service

-Users must be able to access NYSC Transportation vehicles independently or with minimum assistance

Please note that NYSC Transportation vehicles are not wheel chair accessible.

Location: Transportation can be arranged as follows:

• To and from medical appointments

• To and from Adult Day Services

• To and from Senior Adult Centre

• Wednesday and Friday rides to and from Centerpoint Mall for the Shopping Program

All rides must occur within NYSC service catchment area, with the following exceptions:

• Branson Hospital

• Baycrest Centre for Geriatric Care

• Sunnybrook Health Centre

• Toronto Rehabilitation Centre

Schedule: Requests for rides should be made through the Transportation Department. Drivers will not accept bookings for rides.


Ladies Dine

A support service which provides transportation, a nutritious meal and a social activity to lady clients who would not be able to attend without assistance. Clients who are frail and live in conditions of isolation are eligible to participate in this program.

Location: Ladies Dine takes place at 21 Hendon Avenue.

Schedule: Monday and Friday 11 AM to 2 PM.


Friendly Visiting

A support service which provides a volunteer on a one on one basis to visit an isolated or homebound senior. Friendly Visiting and Telephone Reassurance offer an opportunity to monitor the general well being of our clients.

Location: In the client’s home.

Schedule: To be co-determined by the volunteer and client.


Security Check

A support service providing isolated seniors with regular contact to determine that the client is not in a crisis or at risk of harm. Volunteers are assigned to perform daily safety checks through a short phone conversation.

Location: By phone.

Schedule: This service may be offered up to 7 days per week depending on clients needs.


Homemaking

A support service provided by Community or Personal Support Workers to assist clients with light housekeeping, laundry, personal care, meal preparation, grocery shopping, escorting clients to appointments and some local errands. Clients 55 years of age and older or adults with physical disabilities residing in NYSC catchment service area are eligible to receive these services.

An NYSC Case Coordinator will complete an assessment in the client’s home to identify the specific needs of the prospective client.

Location: In the client’s home.

Schedule: Services are available 24 hours a day, 7 days a week.


Information and Referral to Other Organizations

In the event NYSC cannot provide all services required by each individual client, NYSC Case Coordinators will assist clients and their caregivers to access resources through other service providers in the community.

Some of the organizations that collaborate with NYSC to provide appropriate support to clients are:

Meals on Wheels and More, COTA, Family Services Association of Toronto, North York Seniors Health Centre, CCAC, Neighbour Link, Care Ring, SPRINT, Don Mills Foundation, Downsview Services for Seniors, Circle of Care and many others.

Location: On the phone or in person, in the course of the consultation with a Case Coordinator.

Schedule: 8:30 AM – 4:30 PM


Case Coordination

NYSC Case Coordinators are available to adults 55 years of age and older and physically disabled adults living in the NYSC service catchment area.

Our Case Coordinators work in co-operation with client and their caregivers to:

• determine the services that will best support the individual clients enabling them to remain in their home

• ensure co-ordination of the services accessed by clients through NYSC

• provide assistance in acquiring services offered by other agencies in the community.

Location: N/A

Schedule: Throughout clients care

Priority will be given to clients requesting transportation for medical appointments, Adult Day Services and to clients whose registration file indicates a Priority Classification. This classification is based on the physical, emotional and financial assessment of each client.
